Rashi explains the anomaly that only by leprosy
is declaration of a priest required for the status
of ritual impurity:
All other ritual impurity statuses are objective; they
are consequences of the conditions of that particular
impurity. However leprosy impurity, besides requiring
the presence of the conditions of leprosy impurity, also
require a verbal pronouncement by the priest. Without
this pronouncement the person is not yet ritually impure
even though the conditions for impurity are present.
